What to expect from our services

Our step by step guide tells you what to expect from initially contacting our services through to continuing care. 

  1. Referal

    Find out how to make an initial referal to us. 

  2. First contact

    Find out who will contact you and what they could ask. 

  3. Allocation to a worker

    Our teams work with different people depending on their situation and needs. 

  4. Social care assessment

    When you meet your worker they'll get to know you and your needs. 

  5. Carers assessment

    Anyone providing unpaid care will be offered a carers assessment. 

  6. Eligibility decision

    If you have eligible needs, we will work with you to create a support plan. 

  7. Support plan



    A support plan will set out what support you need, when you need it, and who will provide the support. 

  8. Financial assessment

    We will assess to see what we could contribute towards your ongoing care. 

  9. Review

    We aim to review new plans within 6-8 weeks and then at least yearly.
