We review new support plans within six to eight weeks of support being put in place. They'll be reviewed yearly after that.
A review can be ‘light touch’, taking place on the phone or a brief home visit.
A full review might include a ‘functional assessment’ of how you are managing certain day to day tasks. This would consider whether there are ways to support you to be more independent.
If your needs or circumstances have changed, a new social care needs assessment will take place.
If your situation has changed, you can request a review by contacting your allocated worker or get in touch with the team which oversees your care if you do not have one.
