Allocation to a worker

Depending on your situation and needs, you would be referred to a worker in one of the following teams:

  • Occupational Therapy – to assess for equipment and adaptations.

  • Gateway Duty - if your needs are urgent or your situation can be improved quickly

  • Whole Life Disability Service- if you have a diagnosed learning disability.

  • Mental Health Social Work Team- if you are open to NHS secondary mental health services. For example, Early Intervention, Community Treatment Team, or addiction recovery services.

  • Older People’s Mental Health Social Work Team – if you have a care coordinator from the NHS Mental Health Services for Older People team.

  • Community Wellbeing Teams - if you have a longer-term need for support and are not eligible for our specialist mental health or learning disability teams.

Will I need to wait?

If there are high numbers of referrals, you may need to wait for a named worker to be assigned to you.

If we can't give you a worker straight away, we will assign you a priority rating. This will depend on the level of risk to your health, safety and wellbeing.

Work is allocated on the basis of priority rating and the length of time someone has been waiting.
