Early Years SEND Surgery ‘Plus’

What is it?

SEND Surgery Plus is used to track and monitor children whose needs were identified following referral through a ‘Child Alert’ referral process and discussion at the EY Partnership triage panel.

The SEND Surgery Plus is a 45-minute appointment via TEAMS with the EYIS SENDCo to discuss the child’s needs in depth. 

 

Why is it needed?

To ensure that children reach Reception age with their needs being recognised and supported. 

Some children have complicated needs that may require additional support and resources other than that which is ordinarily available.  This support will need to be carefully considered and planned for. 

 

Who will the information be shared with?

We may need to discuss your child’s information with partners.   Sharing this in education, health, public health and care will help us to identify the most appropriate support service to meet your child’s individual needs.  A record of the SEND Surgery Plus will be sent to the Early Years Setting and a copy will be held with the local authority. 

 

What will happen next?

  • The outcome and next steps will depend on whether the child needs to return to the Early Years Partnership Triage Panel for appropriate service allocation.

  • A record of this discussion will be held with the Local Authority.
